To report any concerning behavior, sexual misconduct, harassment, conduct issues, crimes, accidents, bias incidents, discrimination, or general safety concerns, please select the "File Report" button located at the top of this page.

This reporting form is not a substitute for contacting 911 or emergency services. Do not use this site to report events presenting an immediate threat to life or property. If you require emergency assistance, please dial 911.

Incident Reporting Form

When should you use this form?

BTC students, employees, and visitors should use this form to report incidents of:

  • Accident/safety violation(s)
  • Behavior involving bias or discrimination
  • Criminal activity
  • Gender discrimination
  • Hazing
  • Misconduct
  • Relationship violence
  • Sexual misconduct (includes sexual harassment, sexual intimidation, and sexual violence)
  • Stalking
  • Other concerning behavior(s)

BTC employees (including student employees) may also use this form to report incidents of discrimination and harassment based on a protected status (e.g., race, ethnicity, creed, color, sex, gender, citizenship status, national origin, age, marital status, religious preference, the presence of any sensory, mental, or physical disability, reliance on public assistance, sexual orientation, veteran status, political opinions or affiliations, or genetic information).

A student who would like to report alleged cheating or plagiarism should speak to their faculty. Faculty should report alleged cheating or plagiarism using the Academic Integrity Violation Form available on the Instructional Reporting page of the Instruction Intranet site.
